Eligibility

If you lose your Identity Card (IC), you need to report the loss to ICA and apply for a replacement in person within 14 days. If you are a victim of crime, e.g. theft, you will need to file a police report. If your IC was lost for any other reason, you can fill out a statutory declaration when you report the loss.

If you later find your lost IC after receiving a replacement, you must return the found IC to ICA.

Documents Required

You will need to provide original documents and official translations (for documents in languages other than English), as well as copies of the documents for ICA to retain.

ICA accepts:

  • Translations provided by the embassy of the country that issued the document
  • Translations produced by a notary public in Singapore or the country that issued the document
  • Privately created translations attested by the embassy of the country that issued the document, or notarised by a notary public in Singapore or the country that issued the document.
  • ICA does not endorse any private translation companies or entities. Insufficient documentation may result in non-acceptance of your application. You may be required to provide additional documents to support your application.

    To apply at the counter, you will need to provide the following documents ready:

    • Your recent passport-sized colour photograph, taken within the last three months. Please click here for photo guidelines.
    • Police Report or Statutory Declaration 
    • Baptism or religious certificate (if you want to include a religious name)
    • Deed poll for a change of name, including an ethnic name (if applicable)
    • Marriage certificate (if you want to include your married name).

    If you are a permanent resident, you will also need to provide the following additional documents:

    • Entry and Re-Entry Permits
    • Expired and valid passports
    • Certificate of Identity (if any)
    • Malaysian IC (if applicable).

    Operating hours are 8.00 am to 4.30 pm on Mondays to Fridays, and 8.00 am to 12.30 pm on Saturdays.

    Longer-than-average waiting times are to be expected on the following peak times:

    • Mondays, Fridays, Saturdays
    • School holidays
    • The day before or after a public holiday.
    Fees

    You will need to pay the following fees:

    • S$100 for first loss
    • S$300 for second and subsequent losses.

    Payment can be made using NETS, NETS FlashPay or CashCard.

    Processing Time

    First loss: one month

    Second and subsequent losses: three months

    Collection

    The date of collection will be indicated on your collection slip after payment.

    In-person collections must be done with the original collection slip at:
