Overview

Singapore Citizens and Permanent Residents who have lost their Identity Card (IC) are required to report the loss and apply for a replacement in person within 14 days. A police report is required only if you are a victim of crime. Otherwise, you may execute a Statutory Declaration when you come to ICA to report the loss.

If the registrant subsequently finds the lost IC after receiving a replacement, he/she must return the found IC to ICA.

Documents Required

To report loss of IC, the documents required are:

  • One recent passport sized photo which meets the following requirements:
    • The photograph must be taken within the last 3 months
    • The photograph should be in colour, must be taken against a white background with a matt or semi-matt finish
    • The photograph image must show the full face and without headgear (headgear worn in accordance with religious or racial customs is acceptable but must not hide the facial features)
    • The photograph cannot be taken in uniform
  • Police report or completed Declaration Form NR 12 (PDF). A police report is only required if you are a victim of crime. Otherwise, you may execute a Statutory Declaration when you come to ICA to report the loss.

Additional Documents Required for Permanent Residents

  • Entry & Re-Entry Permits
  • Expired & valid passports
  • Certificate of Identity, if any
  • Malaysian IC, if applicable
Procedure
  • Call in person with the relevant supporting documents at:

  • Take a Q-number and wait to be called
  • Produce the original copy of the police report, if available
OR
  • Produce completed Statutory Declaration (Form NR12)
  • Make payment ($100 for the first loss and S$300 for subsequent losses) and collect the receipt
Note: The receipt must be produced when collecting the replacement IC (in person) in 1 or 3 months’ time

Fees

First loss : $100
Second and subsequent losses : $300

Payment can be made by NETS or CashCard.

Processing Time

First loss : 1 month
Second and subsequent losses : 3 months

Peak Periods to Avoid
  • Mondays, Fridays, Saturdays, school holidays, eve and the day after public/school holidays
  • After 2.30 pm on weekdays
  • After 11.00 am on Saturdays and school holidays
There is generally a larger crowd in March, end May, June, end November and December
