Overview
A lost passport must be reported immediately to the Passports Section, Citizen Services Centre, Immigration and Checkpoints Authority or to the nearest Singapore overseas mission abroad.
Passports reported lost cannot be
used even if recovered subsequently. If you recover the lost passport after a new one has been issued to you, you must surrender the recovered passport to ICA for cancellation within 14 days. Failure to surrender a recovered passport is an offence punishable by a fine not exceeding S$1,000 or imprisonment for a term not exceeding 6 months or both.
Documents Required
For Passports Lost in Singapore
- Police report* or Statutory Declaration at ICA Building
- Completed Application Form for Loss of Singapore Passport (PDF)
- Completed Supplementary form for first loss of Singapore Passport (PDF);
or Completed Supplementary form for second loss of Singapore Passport (PDF);
or Completed Supplementary form for third/subsequent loss of Singapore Passport (PDF).
For Passports Lost Overseas
- Local police report*
- Documentary proof of Singapore Citizenship (e.g. Identity Card)
- Passport-sized photographs
* A police report is only required if you are a victim of crime. Otherwise, you may execute a Statutory Declaration when you come to ICA to report the loss.

For Passports Lost in Singapore
- Obtain a Q-Number at Passports Section, Citizen Services Centre:

Saturday : 8.00 a.m. to 12.30 p.m.
Note: By law a lost passport has to be reported within 14 days of the loss, but it is strongly advised to report it immediately.
For Passports Lost Overseas
- Make a police report in the foreign country and request for:
- a copy of the report (preferred) OR
- acknowledgement slip/receipt OR (if the above two options are not available)
- report number & name/location of police station
- Report the loss to the nearest Singapore overseas mission with the relevant supporting documents
- Collect the Document of Identity (which will facilitate your return to Singapore) from the Singapore overseas mission
- Apply for a replacement passport in person at ICA upon your return to Singapore
